Your First Visit

We appreciate that you have selected us to provide your neurological care. To begin this process, we will need some patient paperwork completed. You can obtain our forms online at our Patient Information Center, complete them at home, and bring them with you. Alternately, for patients without internet access, you may call our office and we will mail or fax materials to you.

Please be sure to bring your insurance card to your first appointment.

In addition, if your insurance requires a referral form, it is your responsibility to make sure that your primary care physician has followed the appropriate steps to get that done before your visit. Please check on this to avoid having to re-schedule your appointment with us.

We will also need any chart notes and previous tests, including radiology studies (such as MRI and CT scans) or nerve conduction tests. If you don’t have these, please ask your doctor’s office to send them to us.

• During office hours, you may leave a telephone message if we are helping other patients, and we will return your call promptly.
• If you have something urgent after hours, call our office and our service can contact the doctor for you.
• If you have a medical emergency, even during office hours, call 911 or go to the emergency room.
